什么是睾酮?它有多重要?

睾酮是一种类固醇激素,男性从青春期开始终生由睪丸分泌。然而从30岁左右开始,睾酮水平会随年龄增长以每年1-2%的速度下降。不良生活习惯可能加速这个过程,例如过量饮酒、睡眠不足、缺乏运动、压力过大和饮食不均衡。

 

睾酮不仅负责调节性欲(性冲动),还影响骨密度、体脂率、肌肉量以及红血球的生成。当睾酮水平低于正常值时,可能导致肌肉量减少、性欲低下、易怒、抑郁、注意力不集中、体毛脱落、疲劳以及骨质疏松风险增加。

 

需特别注意的是,性欲并非仅由激素驱动。若生活习惯失衡,比如运动过量或不足、酗酒或存在抑郁/焦虑等心理问题,这些都可能影响性欲。因此找出根本原因至关重要,而非简单归咎于睾酮不足。

 

为何要检测性激素结合球蛋白(SHBG)?

所有激素都需要载体蛋白在体内运输。睾酮主要通过SHBG和白蛋白在体内传输1 。 SHBG与睾酮的结合能力远高于雌激素2 ,其主要作用是控制性激素对细胞的可用性并进行全身运输。要确定有多少睾酮能进入细胞(即「游离睾酮」),需通过SHBG与总睾酮水平的差值计算。现代实验室会自动进行这项计算来评估游离睾酮水平。相较总睾酮值,游离睾酮能更准确反映睾酮状态。3, 4

 

SHBG正常值范围为10-57mmol/L。男性随年龄增长常见的状况是SHBG水平升高,从而阻碍游离睾酮被细胞利用。 SHBG过高的症状与前述低睾酮症状相似。5

 

因此我们需要找出SHBG升高的原因来降低其水平。对男性而言可能与甲状腺机能亢进6 或生长激素不足有关7。肝脏问题也是潜在因素,因为SHBG由肝脏产生,若患有非酒精性脂肪肝、肝炎或肝硬化等肝脏疾病,SHBG可能偏高。8,9.

 

生活习惯同样影响SHBG水平,包括酗酒10, 、吸烟11, 和过度压力12。这种压力也可能源自过度训练。

SHBG过高的长期影响

研究显示SHBG过高可能带来长期疾病风险,包括骨质流失与骨密度下降13,14. ,同时增加阿兹海默症15 和前列腺癌的患病风险。16

 

如何降低SHBG?

饮食调整能有效改善:高蛋白饮食有助降低SHBG17. 。建议蛋白质基础摄入量为每公斤体重1.2克,运动量较大者需适量增加。初步可透过肝功能检查确认总蛋白水平。

 

将体脂率降至健康水平也能降低SHBG18 :女性建议低于22%,男性建议低于20%。

 

某些矿物质和维生素补充剂也能降低SHBG,其中硼元素效果最显著,效果最显著,研究建议每日摄入10mg可降低SHBG19. 。最佳补充形式为甘氨酸硼或柠檬酸硼。需注意的是,若总睾酮水平原本就高,SHBG自然会高于正常值,此时建议从低剂量(3-6mg/日)开始补充,2-4周后检测游离睾酮水平。理想值应维持在2-4%之间。

 

维生素D3能降低SHBG并提升睾酮,最适水平约50ng/dL。硼与维生素D3具有协同效应:硼能延长维生素D3在血液中的半衰期,增强其作用效果。鱼油同样能降低SHBG20 ,最安全的Omega-3摄取方式是食用沙丁鱼、鲭鱼、野生鲑鱼、鲱鱼或凤尾鱼等鱼类,每周四份即可(罐装产品Omega-3含量相当或更高)。

With Father’s Day this week, we’re looking into one of the most common complaints by men as they age – hair loss. Here are 7 functional remedies for hair loss. If you haven’t seen our previous article, check it out for the common causes of reversible hair loss.

Detoxification

As toxins have been linked to hair loss, evidence-based detoxification can remove the triggers for hair loss and begin letting hair regeneration occur. The liver doesn’t just clear out toxic compounds, it also clears excess hormones – such as excess DHT known to cause a large percentage of male hair loss cases.

  • Chelation therapy works by introducing a chelator to the body, usually through an IV. Chelators (like DMSA and EDTA) are compounds that act as a sponge to soak up heavy metals before safely excreting them in your urine.1 Proper chelation should be low and slow to reduce the common side effects of detoxification like fatigue, brain fog, mood disruption, sleeping issues and more. Just be sure to include nutrient replenishment afterwards, as chelation can strip some of your essential minerals along with it.
  • Infrared sauna therapy raises your internal temperature to induce a sweat response. Sweating is an ancient therapy that is now proven by science to help the body rid itself of heavy metals including lead, mercury, arsenic and cadmium.2

You can’t build anything without the nutrients to do so, whether it’s muscles, long nails or thick hair. Hair loss is extremely common, not just later in life, but in our 40’s, 30’s, and even 20’s. Thankfully, many types of hair loss are reversible. 毒性物质暴露

根据2007-2017年间47项研究的系统性回顾,与掉发相关的毒性物质包括:铊、汞、硒、秋水仙素、硼酸、砷,以及某些合成鸦片类药物。2

缺乏营养素

营养素是细胞的基本建构材料,因此营养不良与发量稀疏、发质变差密切相关。值得注意的是,头发生长在人体生理机能中属于次要功能,唯有当身体处于健康状态时,才能维持茂密生长。维持头发健康的关键营养素包括:铁、锌、烟碱酸、硒、生物素、维生素A和D3 。此外,低蛋白饮食也会导致掉发,因为头发95%由角蛋白构成。

药物副作用

不仅强效的化学治疗会造成掉发。研究显示,即使是抗生素、避孕药,甚至常见的止痛药如布洛芬等看似温和的药物,也可能引起暂时性掉发。4

荷尔蒙失衡

荷尔蒙失衡会全面影响头发的生长、强韧度与光泽度。甲状腺激素失衡会导致头发变得细软脆弱,而过高的睾酮水平(特别是二氢睾酮DHT)会促使毛囊萎缩并缩短生长周期5 。值得注意的是,许多常见的生发药物正是通过阻断DHT受体来发挥作用。6

真菌过度繁殖

真菌特别容易在温暖潮湿的环境中滋生。念珠菌这类真菌尤其容易失控繁殖,特别是在摄取过多糖分或长期处于压力状态下。当体内念珠菌过度生长时,头皮可能出现发红、发炎、搔痒和脱屑等症状,进而导致局部性掉发。7

这个问题值得每个人深思。无论你是40岁还是80岁,只要代谢不健康或出现代谢综合症征兆,患病风险就会显著增加。维持代谢健康更能有效降低患上慢性疾病的机会,包括心脏病、糖尿病、癌症及认知障碍症等。

一般来说,代谢健康评估是透过年度身体检查进行的,医生会根据以下5项标准来判断代谢健康状况。 2002年美国国家胆固醇教育计划成人治疗小组(NCEP ATPIII)制定的代谢综合症定义标准包括:

  • 空腹三酸甘油酯水平超过150 mg/dL
  • 空腹高密度脂蛋白低于40 mg/dL
  • 空腹血糖超过100 mg/dL
  • 腰围超过40吋(男性)或35吋(女性)
  • 血压达130/85 mmHg或以上

若符合其中3项,即被界定为代谢综合症,亦即「代谢不健康」。代谢涉及身体多个关键系统,透过分析葡萄糖和脂质代谢,我们能预测一个人未来患上癌症、心脏病和糖尿病等慢性疾病的风险。但从功能医学角度来看,现行标准存在重大缺陷——大多数人在检测时都能达标,造成健康假象,实际上这些参考值设定得过于宽松。

空腹三酸甘油酯

以空腹三酸甘油酯为例,超过150 mg/dL才被视为异常,这个标准实在过高。高三酸甘油酯血症(即血液脂肪水平过高)会加剧胰岛素阻抗和炎症反应。理想水平应维持在100 mg/dL以下,以减轻对血管的炎症和氧化损害。

空腹高密度脂蛋白

高密度脂蛋白低于40 mg/dL即被归类为偏低,但这种水平与心血管疾病发展密切相关,特别是当三酸甘油酯和低密度脂蛋白同时偏高时。理想水平应达60mg/dL或以上,才能充分发挥高密度脂蛋白的保护作用——将低密度脂蛋白从血液运回肝脏,减少其氧化变质的机会。但这些保护作用只有在三酸甘油酯和低密度脂蛋白均低于100时才有效。

更具参考价值的是高密度脂蛋白/总胆固醇比率,以及三酸甘油酯/高密度脂蛋白比率。前者应大于0.24,比率越高越好;后者若超过4即属偏高,这个比率是预测心脏病最有力的指标之一。研究显示「三酸甘油酯/高密度脂蛋白比率最高的人群,患心脏病的风险是最低比率者的16倍」。

空腹血糖

空腹血糖标准100mg/dL同样设定过高。研究指出,空腹血糖超过95mg/dL的人,未来患糖尿病风险是低于90mg/dL者的3倍。从功能医学角度,我们建议将空腹血糖控制在85mg/dL以下为佳。

腰围

虽然男性40吋、女性35吋的腰围标准能反映代谢风险,但更重要的是测量内脏脂肪含量。内脏脂肪会分泌影响代谢疾病风险的激素,其中脂联素和瘦素是两个关键指标——理想状态是脂联素水平高而瘦素水平低。研究证实高瘦素水平与动脉粥样硬化和心脏病密切相关。

要准确测量内脏脂肪需要进行电脑扫描或磁力共振检查,简易方法是计算总体脂的10%。体脂分析仪显示内脏脂肪健康水平应低于13%,超过此数值即属不健康,建议立即调整生活习惯。
